Hammersmith and Fulham, West and Central London Clinical Commissioning Groups are holding an event for people with long term conditions like diabetes, dementia, chronic heart disease (CHD) and chronic obstructive pulmonary disease (COPD) to help design a better way to help them manage their self-care.

They are also asking carers, voluntary and community groups, and health and social care professionals.

There will be three events repeated across West London:
